3ds club nintendo registration

I was registering my Nintendo 3ds and than my computer crash so after checked my system list it show that my Nintendo 3ds was register but I didn't got any coins or survey to fill the only thing I got is the linked eshop and club Nintendo reward and because of that I didn't got my gold member reward is there a way to fix this issue?

Look in your To-Do List. You should find the Nintendo 3DS's registration survey there, and when you fill it out and send it, you'll get your coins.

I suggest that you call Nintendo for assistance with this issue. You can call them at 1-800-255-3700, from 6 a.m. to 7 p.m. Pacific Time.
